BSW Social Worker Job 2024 Full Time

* This is a fully remote position. Currently looking for part-time or PRN.

POSITION SUMMARY:  

The Social Worker Care Manager is an integral team member for serious illness services and focuses on the non-medical aspects of care, such as barriers to care associated with social determinants of health. Barriers to care can comprise as much as 80% of avoidable hospitalizations, and negatively impact quality of life for our members. The Social Worker Care Manager assesses, plans, and coordinates care across the continuum to promote quality, cost effective care consistent with the member’s values and goals. 

ESSENTIAL DUTIES & RESPONSIBILITIES: 

• Responsible for performing initial and ongoing psychosocial assessments with a holistic perspective that incorporates identification of SDoH barriers to care and psychosocial needs. 
• Assist client/family/caregiver with determining health care goals. 
• Develop plan of care based upon assessment with specific objectives, goals and interventions designed to meet member’s needs and their priorities. 
• Provide patient and provider education. 
• Facilitate member access to community-based services.  
• Monitor referrals made to community-based organizations, medical care, and other services to support the members’ overall care management plan.  
• Contribute to patient and family supportive counseling and education with regards to family systems/dynamics and coping method

• Knowledge of Federal, State and Local regulations  
• Knowledge of HIPAA privacy rules and regulations 
• Knowledge of electronic medical records systems 
• Ability to conduct psychosocial assessments via remote technology, without having visibility into non-verbal cues.

OTHER DUTIES:

• Practice within the scope of license, certification, or registration, where applicable, and as required by law and regulation. 
• Develop and maintain professional relationships with all members of other teams involved in client’s care. 
• Demonstrate the ability to function in a variety of care environments, including home settings, skilled nursing facilities, assisted living environment, etc. 
• Responsible for obtaining the necessary information from third party providers for payment. 
• Responsible for facilitating internal referrals. 
• Assist in providing after hours services if necessary. 
• Attend staff and mandatory in-services meetings, as required. 
• Participate in ongoing education and assume responsibility for updating professional skills. 
• Practice safety and infection control methods 
• Perform other, yet similar duties as assigned. 

MINIMUM QUALIFICATIONS:  

Education:  

Bachelor’s in Social Work (BSW) 

Experience:  

1-3 years of experience within a healthcare setting 

Licensures & Certifications: (if applicable) 

Valid NC Social Work Certification/Licensure, either CSW, CMSW or LCSW 

APHSW-C if preferred, but not required

Job Type: Part-time

Pay: $60,778.00 – $66,224.00 per year
