In home medical care for homebound patients is proven to increase patient comfort, improve health outcomes, reduce health care costs and increase provider/care team satisfaction. With the population aging, more people are suffering from multiple chronic conditions and degenerative diseases. This trend will continue as roughly 10,000 baby boomers a day enter the Medicare system.

General Statement of Duties

The Quality Improvement Data Specialist is responsible for gathering, synthesizing and reporting data. Additionally, this position monitors relevant information sources to anticipate and operationalize needed changes to departmental and organizational workflows as regulatory requirements and information needs change. This position supports identification and maintenance of quality efforts for clinical operations to meet regulatory requirements and accomplish business objectives, including value/performance-based revenue goals.

Essential Position Functions

Data Management and Reporting

• Perform data collection, synthesizing and reporting as required for payers and other programs including Medicare and Medicaid; identify gaps and coordinate with providers.
• Generate routine and ad hoc reports across the organization.
• Liaise with commercial and government agencies responsible for the development of quality metrics; assist with problem solving and recommendations.
• Suggest and contribute to new workflows to improve clinical and support staff understanding, partnership, and engagement with quality measures.
• Coordinate the implementation of workflows that support documentation requirements necessary to meet quality measures.
• Track measurable performance thresholds for quality indicators to evaluate workflow effectiveness and to determine the infrastructure required to support quality standard needs; provide input toward improvements.
• Collaborate with other healthcare clinics/delivery systems (e.g., OCHIN, ALERT IIS) to identify opportunities to reduce quality program costs while achieving outcomes.
• Develop and administer data collection and survey tools.
• Analyze data to identify trends and report to relevant leadership.

Compliance Tracking

• Develop and maintain knowledge and understanding of commercial and governmental value/performance-based payment model regulations.
• Monitor and maintain knowledge of quality and compliance requirements.
• Remain cognizant of changing program requirements and work to proactively implement necessary changes to departmental and organizational workflows as needed.
• Remain abreast of relevant changing benchmarks and thresholds.

Essential Department and Organizational Functions

• Maintain patient/family and staff confidentiality in compliance with HIPAA laws.
• Meet deadlines for completion of workload.
• Maintain agreed upon work schedule and meet deadlines for completion of work.
• Serve as an ambassador for CareOregon and Housecall Providers at all times.
• Demonstrate cooperation and teamwork and maintain collaborative relationships with all levels of staff and community partners.
• Attend staff and mandatory in-services meetings, as required.
• Participate in ongoing education and assume responsibility for updating professional skills
• Perform other related duties as assigned.

Knowledge, Skills and Abilities Required

• Excellent attention to detail and follow through on completion of tasks
• Ability to coordinate information, processes and workflows
• Ability to work independently, prioritize tasks, and maintain focus on organizational goals
• Ability to propose and implement solutions to identified problems
• Proficient in MS Word and Excel, with ability to organize and track data; reporting skills preferred
• Excellent communication skills with the ability to handle and communicate sensitive information
• Excellent relationship development skills with the ability to work with teams to effect improvement in patient care.
• Knowledge of quality improvement concepts, roles, processes, and reporting and HIPAA privacy rules and regulations preferred
• Ability to adhere to organizational standards, policies and procedures

Education and/or Experience

Required:

• Minimum 2 years’ experience within the health care industry, including exposure to medical terminology and records and 1 year of experience in a quality related field

Preferred:

• Experience or training using Access or other quantitative software program
• Experience with quality data collection and reporting
